These Terms of Service ("Terms") govern your use of PatientTrac Revela, an electronic medical record platform for plastic and reconstructive surgery, operated by PatientTrac, LLC ("PatientTrac", "we", "us").
Revela is a professional healthcare software platform intended exclusively for licensed healthcare providers, authorized clinical staff, and their administrative personnel. Access is granted only through authenticated sessions issued by PatientTracForge or a verified Revela administrator.
AI-generated content in Revela (clinical flags, procedure suggestions, operative note drafts, cost proposals) is provided as decision support only and does not constitute medical advice. Physicians and licensed providers retain full clinical responsibility for all patient care decisions. AI outputs must be reviewed and validated by a qualified clinician before acting on them.
All users agree to comply with HIPAA, HITECH, and applicable state privacy laws when accessing, entering, modifying, or transmitting PHI through the platform. Users must report any suspected breach or unauthorized access to security@patienttracforge.com immediately.
Patient clinical data entered into Revela remains the property of the covered healthcare provider (the subscribing practice). PatientTrac holds a limited license to store, process, and display that data solely to provide the contracted services. We do not sell patient data.
In compliance with ONC 21st Century Cures Act information blocking rules, Revela supports patient and provider access to Electronic Health Information (EHI) in standard formats. Export requests should be directed to your practice administrator.
To the maximum extent permitted by law, PatientTrac's liability for any claim arising from use of Revela is limited to the subscription fees paid in the 12 months preceding the claim. PatientTrac is not liable for indirect, incidental, special, or consequential damages.
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Disputes shall be resolved by binding arbitration in Miami-Dade County, Florida.
